Class will focus on accidents, injusries, near misses, fires, explosions, equipment damage, spills, community and environmental damage. Learn to conduct an investigation – team charter and make-up; getting the facts – listening, questioning, obseving, collecting evidence; using logical analysis to determine primary, secondary and potential causes; develop sound conclusions and recommendations; report writing. Students will condust investigations in class using historical incidents OR they may bring in a case history from their company to analyze the alternative approches.

All companies, under the General Duty Clause of OSHA must analyze and
reduce potential hazards (equipment, facility, environment, etc…) to
their employees to create a safe working environment. OSHA and EPA
specifically require companies working with dangerous materials that
present a risk to employees, the community, and the environment to
conduct hazard risk analysis. Learn to conduct hazard and risk analysis
using accepted and recognized procedures and techniques; determine the
appropriate hazard analysis for you company; conduct the analysis,
determine the solutions, evaluate risks, prioritize the solutions based
on use of limited resources and document the results.
